Please save the bus pass file to your computer. Fill out the appropriate boxes. Re-save the file as your child's name and then choose a way to send in the form:
You may send in the form with your child in his/her communication folder.
You may email it to Ms. Lauria before 10 a.m. at Elizabeth.Lauria@cobbk12.org or
You may fax it to Eastvalley at 770-578-7216.
